Railroad Crossing Case -

There were five significant human factors issues in this case.

  1. The long building obscured the train as the motorcycle approached the crossing.
  2. A crossing gate would have been more perceptable to the motorcycle driver.
  3. The sun was directly behind the signal light and made it difficult to see.
  4. The traffic light at a nearby intersection was not interlocked with the signal light so the light at a nearby intersection turned green even though the train was still approaching.
  5. 12 inch diameter red signal lights should have been used. What were in place at the time of the accident were 9 inch diameter signal lights that contained 25 watt bulbs. The low wattage was to preserve battery life.

The motorcycle passenger lost both legs under the wheels of the train.
