Crossing Case -
There were
five significant human factors issues in this case.

- The long
building obscured the train as the motorcycle approached the crossing.
- A crossing
gate would have been more perceptable to the motorcycle driver.
- The sun
was directly behind the signal light and made it difficult to see.
- The traffic
light at a nearby intersection was not interlocked with the signal light
so the light at a nearby intersection turned green even though the train
was still approaching.
- 12 inch
diameter red signal lights should have been used. What were in place
at the time of the accident were 9 inch diameter signal lights that
contained 25 watt bulbs. The low wattage was to preserve battery life.

The motorcycle
passenger lost both legs under the wheels of the train.
