Exploding Grinding Wheel
An employee of a machine shop placed a grinding wheel on a lathe to grind the outside diamater of a part. Due to the lack of proper warnings and instructions he used too large a grinding wheel for the operating speed he was utilizing. As a result, the grinding wheel exploded and one of the pieces hit him in the chest resulting in his death.
